Discovering Potential Program

Participant Intake Form

This form is to be completed and submitted by the youth participant for the purpose of applying for the Discovering Potential wage and training subsidy program administered by Electricity Human Resources Canada. Your employer or training provider will also be required to submit a Company Intake Form.

• Résumé of participant

The Participant Must

• Be a youth between the ages of 15 and 30 years of age at the start of the program
• Be a Canadian citizen, permanent resident, or person who has been granted refugee status in Canada
• Be legally entitled to work in Canada
• Be legally entitled to work according to the relevant provincial legislation and regulations

The participant may be receiving EI while participating in the program. *The applicant must first consult with their Service Canada Office before participating.
